Go to https://modo.us/amoe while logged in to generate the 12-digit Envelope Request Code. Modo also generates the exact one-time handwritten statement on that page; copy that statement onto the card instead of using a generic statement.
AMOE requirements
- Request code
- Unique 12-digit Envelope Request Code required. Generate it from https://modo.us/amoe while logged in. The code is unique, can only be used once, expires 30 days after display, and can only be used by the account that generated it.
- Request card
- Standard white, one-sided, unfolded, unlined 4x6 index card. The company address and all information on the card must be handwritten solely by the participant. The card must be signed and dated immediately beneath the handwritten statement.
- Envelope
- Standard white, one-sided, unfolded, unlined #10 envelope, posted from the same state as the participant's verified residential address.
- Card text
- Full name as shown on government-issued identification
- Email address registered to the Modo account
- Residential address registered to the Modo account
- 12-digit Envelope Request Code
- Handwritten statement exactly as stated on the Envelope Request Code page
- Signature immediately beneath the handwritten statement
- Date of signature in MM/DD/YYYY format immediately beneath the handwritten statement
