Mail the #10 envelope within 3 days of signing, from the same state as your verified address, and the signature date must fall within the current month. The statement has an [INSERT CURRENT MONTH AND YEAR] placeholder you fill in.
AMOE requirements

- Standard white, one-sided, unfolded, unlined 4x6 index card. All information on the card must be handwritten in the required order. The card must be signed and dated immediately beneath the required statement.
- Envelope
- Standard white, one-sided, unfolded, unlined #10 envelope, from the same state as the verified residential address. The address and all information on the #10 envelope must be handwritten.
- Card text
- Full name as shown on government-issued identification
- Email address registered to the MyPrize US account
- Username registered to the MyPrize US account
- Residential address registered to the MyPrize US account
- The statement: "Sweepstakes Entry! I wish to receive Sweeps Cash to participate in the [INSERT CURRENT MONTH AND YEAR] Sweepstakes Contest offered by MyPrize US. By submitting this request, I hereby declare that I have read, understood and agree to be bound by MyPrize US's Terms of Use and Sweepstakes Rules."
