Critical requirements
Generated code, written as CODE:
You need a unique postcard code from Stake's code generator (one per customer, don't share) - cards without it are rejected. On one side of the card, write the six items in the exact order shown, including the word "CODE:" followed by your code. Use a postcard or white paper at least 4x6, in a stamped envelope, and write 'Stake Cash Credits' plus your return address on the front. Stake.us is 21+. Each valid card is worth 5 Stake Cash.

- Unique postcard code required, generated with Stake's code generator (one per customer; do not share). On the card, write the word "CODE:" followed by your code. Cards without a valid generated code are rejected.
- Request card
- Postcard or white paper Request Card, at least 4x6 inches, unfolded, placed in a stamped envelope. Write the items on one side only, in the exact order shown. Handwriting must be legible (personally handwritten, no stencil or copier). Ink color is not specified in the rules.
- Envelope
- Stamped envelope (size not stated). On the front write your return address and the words "Stake Cash Credits".
- Card text
- Your full name as shown on your government-issued identification
- Your Stake.us username
- The return/residential address registered to your Stake.us account
- The email address registered to your Stake.us account
- The word "CODE:" followed by your unique postcard code
- The statement: "I wish to receive Stake Cash to participate in the sweepstakes promotions offered by Stake Sweepstakes. By submitting this request, I hereby declare that I have read, understood and agree to be bound by Stake's Terms and Conditions."
